The MyIndependence Coordinator - RN delivers quality home healthcare and home care services, ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

Essential Activities and Tasks 

Care Management and Service Delivery - 60%

  • Provides nursing care to resident/patients through the delivery of services to meet resident/patient needs. Follows current home health nursing professional standards of practice and agency approved protocols.

  • Serves as the primary visit nurse for skilled home health patients that reside on the campus.

  • Responsible for the implementation of the physicians’ plan of treatment.

  • Communicates with other Health Care Providers about changes in condition and resident/patient needs.

  • Conducts wellness clinics and service coordination as assigned.

  • Maintains current and accurate resident/patient records in accordance with company policies, federal, state, and local guidelines.

  • Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.

  • Consults with family members and caregivers to promote quality care and appropriate services within the continuum.

Operations Management and Administration - 40%

  • Promotes, educates and collaborates with facility staff on the features and benefits of home health, home care and hospice services.

  • Attends staff in-services and resident events to build trust and relationships with staff and residents. Attends quality, care coordination and aging in place meetings to assist in indentifying clinical and personal care needs.

  • Helps to identify when residents/patients need or qualify for additional services and makes referral to Transitional Care Liaison, Director of Sales and Marketing, or other life plan community resource.

  • Participates in the on-call rotation.

All other duties as assigned.

Qualifications

Education

  • Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.

Experience

  • One year experience in acute care or medical-surgical nursing required. 

  • Experience in a Medicare-certified home health agency preferred.

  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

  • Proficiency with electronic medical records (EMR) preferred.

Other Requirements

  • Must be able to read, write, speak, and understand the English language.

  • Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.

Working Conditions and Special Requirements

  • Sitting - Up to 4 hours/day

  • Standing - Up to 8 hours/day

  • Walking - Up to 6 hours/day

  • Lifting, transferring, pushing or pulling residents/patients or equipment/supplies - Up to 50 pounds

  • Driving - Up to 1 hour/day

  • Work weekends, evenings, and holidays - Required as scheduled

  • Subject to falls, burns from equipment, odors, and cuts - Work day

  • Subject to residents/patients with various disease processes - Occasional

  • May be exposed to infectious waste, disease, conditions, etc. including exposure to the AIDS and Hepatitis B viruses - Work day

  • Risk Category for Exposure to Bloodborne Diseases - I
